All:

Go Canvas

- If you are coming into your worksite you must complete the Go Canvas process and get the green clearance before you enter the building.

- This applies even if you have been vaccinated.

W-2s and 1095

- The 2020 tax forms are available on your Paylocity Self Service Portal

o On the Paylocity portal go the PAY box and click on “more”, then “Tax Forms”

- HR has hard copies, just send us an e-mail indicating you want your forms and we will send them inter-agency to you.

1st Vaccine

- If you tested positive for COVID and have been cleared to work you can choose to get the vaccine at this time.

OR

- If you have changed your mind and now want to receive the vaccine;

- Complete a LFMC Covid Consent Form and send it to HR, we will put you on our waiting list.

Surveillance: This week Dental, Southside and HCH, staff will be responsible for surveillance testing this week. Please don’t expect results over the weekend.

- Beginning times below:

8:00 – 9:00 AM Direct Patient Service: Providers, MAs, nurses, Lab/Pharmacy staff, PSRs, Phone staff

9:00 – 9:30 AM All other Alto Clinic-based staff, including those working from home

Surveillance after the 2nd Vaccine:

- After 14 days of receiving the 2nd vaccine, staff will no longer be required to do surveillance testing.

- All Alto staff who received their second vaccine last week are no longer required to do surveillance testing.

- However, you can contact HR by e-mail if you would like to continue to get tested.
